A. This license is issued to an individual and is not transferable. B. Licenses shall expire on December 31 of each year. C. In order to obtain a license to test milk for payment, an individual must demonstrate testing proficiency by successfully completing a series of split samples provided by the department. After an individual meets the criteria for certification, an application will be filled out and submitted along with payment of a license fee, determined by the department pursuant to Subsection 4-2-2(2), to the department for issuance of a license. |

Split samples may be made available to licensed testers periodically to check for competency in testing. The Department will provide the results to the tester. The split sample results will be evaluated. If a tester does not achieve accurate results, the Department will work with that tester to correct the problem. |

If any provision of this rule or the Utah Dairy Act is violated, the tester's license may be subject to suspension or revocation after due process of a hearing. |
