R434-50. Assistance for People with Bleeding Disorders |

This rule is required by Section 26-47-103 (5). It implements Section 103 of the Health Care Assistance Act, Title 26, Chapter 47. |
| R434-50-2. Definitions |

The definitions as they appear in Section 26-47-103 (1) apply. In addition, "Department" means the Utah Department of Health. |
| R434-50-3. Grant Application |

An applicant responding to a request for grant application under this program shall submit its application as directed in the grant application guidance issued by the department. |
| R434-50-4. Criteria for Awarding Grants |

The department shall consider: (1) the extent to which the applicant: (a) demonstrates that it will provide assistance to the greatest number of persons with bleeding disorders residing across the State of Utah; (b) utilizes other sources of funding, including private funding, to provide bleeding disorder services; and (c) provides: (i) information that meets the requirements established in Section 26-47-103 (3); (ii) a description of the individuals to be served by the grant; (iii) the estimated number of individuals to be served with the grant award; and (iv) the results of an assessment of need demonstrating the need for the bleeding disorder services that the grantee proposes to provide. (2) the cost to the person with a bleeding disorder for the bleeding disorder services; (3) the degree to which the applicant meets the requirements of the statute; and (4) the degree to which the application is feasible, clearly described, and ready to be implemented. |

(1) As required by Section 26-47-103 (1)(b)(iii)(D), the Department establishes that to meet the definition of a person with a bleeding disorder the individual's health insurance must be at or greater than 7.5 percent of the individual's adjusted gross income. (2) The grantee must assure that each individual to whom it provides service under a grant awarded under this rule meets the requirements of this rule and Section 26-47-103 (1)(b). |
